Perioperative Letters
Letter writing is an important method of communication from the Perioperative clinic.
All letters written from the clinic must be sent to :
HNELHD-JHHPeriop@health.nsw.gov.au.
If you require them urgently contact the perioperative office to have them typed on that day. A copy of the letter will be added to the patient’s record on CAP.
The following letter templates are provided as a structure, and can be modified as required. They are in word format and can be edited and saved as required.
